**This is a field-based foster care case management role that will travel and provide coverage to clients throughout Indian River, Martin, Okeechobee and St. Lucie Counties**

Our Communities Connected for Kids Program provides services for children and families in the child welfare system, to ensure a community without abuse, neglect and abandonment. Based out of Port St. Lucie, FL and providing coverage throughout Indian River, Martin, Okeechobee, and St. Lucie Counties, this position is responsible for providing and coordinating the provision of independent living transition services to enable individuals in foster care and young adults previously in foster care to transition to self-sufficiency.  Additional duties include:

 

  • Provide and/or coordinate the provision of independent living transition services to enable older children in foster care and young adults who exit foster care at age 18 to make the transition to self-sufficiency.
  • Provide and/or coordinate independent living services in person and virtually, such as life skills training, educational field trips, and conferences based on the assessed needs of each youth.
  • Provide and/or coordinate life skills services in person and virtually such as, training to develop banking and budgeting skills, interviewing skills, parenting skills, and time management or organizational skills, educational support, employment training, and counseling.
  • Assist young people age 13-17 in licensed foster care or a young adult in extended foster care develop and update their independent living transition plan.
  • Assist young adults in extended foster care develop and update their extended foster care case plan.
  • Assist eligible clients with applying for Road to Independence Program Postsecondary Educational Services & Support (PESS)
  • Assist eligible clients with applying for Education and Training Vouchers (ETV) funding;
  • Educate any parties requesting information on ILP services, such as foster parents, mentors.
  • Link ILP clients with mentors;
  • Coordinate services with other program areas, such as adoptions, behavioral health services, placement and permanency;
  • Participate in the quality improvement process.
  • Attend all applicable staffings for youth 13-17 years of age; attend court hearings as needed.
  • Testify in dependency court proceedings and provide court work to CLS attorneys as needed.
  • Other assigned duties as needed.

 

Qualifications

EDUCATION: 

 

Required:  Bachelor’s Degree in social work or related field in human services or related field from an accredited school.

 

EXPERIENCE:

 

Required: Minimum of two (2) years related experience. Valid Florida driver’s license.

Preferred: Prior foster care transition case management experience. Certification in Child Welfare is preferred.  If not certified, required to obtain credentialing as Child Protection Professional by the Florida Certification Board within 12 months of start date.
