Communities Connected for Kids (CCKids) provides community-based services for children and families in the child welfare system, to ensure a community without abuse, neglect or abandonment. Based out of Port St. Lucie, FL, this position is responsible for the intake of foster care children and placement of those individuals within the CCKids System of Licensed Care. Additional duties and responsibilities include:
- Coordinate with the protective investigators (DCF), case management agencies (CCKids), and child placing agencies placement staff, to ensure children removed from their caregivers are placed in a setting best suited to meet their needs and in a timely fashion
- Ensure children are placed in settings that best suits their needs and receive services that are appropriate.
- Monitor assigned Case Management Agencies, and contracted facilities for quality and accuracy.
- Accept, counsel and deescalate crisis calls, regarding placement issues
- Assist the Protective Investigator or the Case Manager in developing the initial plan of placement action and make appropriate referrals to ancillary professionals, e.g. Certified Behavioral Analysts, Foster Care Support, SAMH, etc., when indicated.
- Assist the Protective Investigator and Case Manager with the placement of children into foster homes, and acute care behavioral settings, as well as provide back-up calls to assist with placement in residential group settings.
- Collaborate with the Family Service Teams and/or case management agencies to provide appropriate services to children and families.
- Assist the Case Managers, when possible and work cooperatively with CCKids subcontractors, regarding the provision of wrap-around services to children and families.
- Assume On-call responsibilities, as assigned by the supervisor on a rotating schedule, to effectively meet Placement needs after normal program hours, weekends and holidays.
- Complete funding requests and forward to the appropriate people for approvals/signatures.
- Participate in the quality improvement process
- Maintain compliance with Minimum Training Requirements (MTR)
- Perform daily FSFN entries, keeping information about child movement in licensed care current, to keep the system of placement and payment to foster homes on track.
